Tonya Flaming is a Case Manager and current owner of Rehabilitation Planning Corp. She has several years of pediatric and youth experience having worked at Sick Kids Hospital as Registered Nurse for 14 years, with the last 6 years spent as a Transitional Care Coordinator. This expertise and experience has allowed Ms. Flaming to excel in her role as a Case Manager in the private industry for over 5 years and has demonstrated understanding, compassion and a client centred approach to working with both paediatric and adult clients, their families and referral sources. Recently Tonya acquired Rehabilitation Planning Corp., a Case Management and Life Care Planning company (www.rehabplan.ca) where she worked as a Case Manager and received outstanding mentorship and experience form Martha Binstock.

Apart from her role as Owner and Case Manager, Tonya currently sits on the Toronto ABI Network Conference Planning Committee as the Sponsorship Chair, as well as on the BIST Awareness Committee (for over 7 years) and completed a 3-year term as a BIST Board member.

    With her background of working at Sick Kids for a decade, Tonya is a wonderful choice for a case manager for patiatric cases. Tonya is very well organized and is a great problem solver. Her experience in the paediatric area often proves invaluable to find innovative solutions to challenging circumstances.

    Me and my family were in a horrible car accident which left my daughter paralyzed we had so many emotions flowing while in sick kids and bloorview and Tonya helped us through it all without having her present through it all I don’t know what would have happened so Hands down Tonya is the best case manager. Whenever I need something for my daughter she gets it done fast!! She has always been there for us and always backs us up when needed!! I couldn’t have asked for a better case manager!! She works very hard for all her clients. When we were in Toronto I could call her at any hour of the day or night and she would answer! She deserves to win case manager of the year!!

    In 2012, my 3.5-year-old son was struck as a pedestrian at 50km/h. After it was clear that he would survive we were informed that he had acquired a catastrophic brain injury and was hospitalized for close to 5 months. It was at McMaster Children’s hospital that I met Tonya Flaming a few days after the accident. Darcy Merkur had brought her in as the team manager for what would eventually become a very robust and effective group of people. Tonya was far more than a team manager, she was a friend, an adviser and a very patient ear to what must have been a terrible thing to watch a family go through. To say that my son’s prognosis was bleak would be an understatement and what he has become is nothing shy of a miracle. I firmly believe it was Tonya’s attention to detail and finding the very best therapist which has made my son’s current life possible. I dropped him off for his first day of grade three earlier this week, which is something that I never thought I would see in 2012. I could not recommend anyone higher than Tonya Flaming. She is incredible at what she does because she cares for each and every person involved an incredible amount.

She is a strong team player and endeavours to ensure positive outcomes for her clients and supporting the families through the recovery process as well.

    Apart from her role as an Owner and Case Manager, she still finds the time to give back to the community, and currently she sits on the Toronto ABI Network Conference Planning Committee and is the Sponsorship Chair (2014 & 2016 and present); as well as on the BIST Awareness Committee for over 7 years and was also a BIST Board member.

    Tonya comes highly recommended as a Case Manager and a professional and is deserving of the Case Manager of the Year award.

She has gone above and beyond with the care she has given Stephanie in the past five years.
